1.	Lost card liability	Customer dispute letter Dully filled and signed claim form Transaction summary by Bank Passport copy (all pages) Police intimation / FIR(First investigation report)
2.	Counterfeit/ Skimming Claims	Customer dispute letter Duly filled and signed claim form by Bank Transaction summary by Bank Axis Bank Club Vistara Forex Card copy Passport copy (all pages) Police intimation / FIR(First investigation report)
3.	ATM assault and robbery	ATM Transaction slip Bank statement showing transaction Vouchers in support of first aid expenses: Original Copy of Police report Copy of identity card
